What is Ablation?
Ablation literally means to destroy. It is an interventional technique used to destroy cancerous tissue in other types of branches, especially in cancer surgery, apart from cardiac arrhythmia. The main field of use in the branch of cardiology is the treatment of heart rhythm disorders, and ablation treatment is one of the most effective treatment methods in heart rhythm disorders today. The main purpose of ablation treatment; it is the elimination of problematic focuses or areas in the heart that cause rhythm disturbances. In this elimination process, either the “burning” process or the “freezing” process is used. The ablation process performed with the burning technique is called as “Warm ablation” and the ablation process performed with the freezing method is called as “cold ablation”.
What is Warm Ablation?
Warm ablation is a form of ablation treatment by applying heat. “Radiofrequency” energy is used in the warm ablation method. Radiofrequency energy, also called radio waves, creates a temperature between 50 and 70 degrees in the target area. This temperature renders the trouble spot or area in the container electrically dysfunctional.
What are the Advantages of Warm Ablation Method?
The biggest advantage of warm ablation is that it completely eliminates the problem causing the arrhythmia. When applied correctly by experienced people, the success rate varies according to the type of palpitation, but it is between 70% and 99%. Ablation success in most types of arrhythmias is almost 100% in experienced centers. The biggest advantage of ablation treatment is that it saves patients from having to use drugs for life.
Let’s come to the advantages of warm ablation over cold ablation or the type of ablation we call “cryoablation”. Warm ablation has two major advantages over cold ablation. One is that the cost is often lower, and the other advantage is that the area removed with warm ablation can be rendered dysfunctional more effectively than with cold ablation. In other words, the burning function can kill the tissue we are trying to destroy more effectively and precisely than the freezing function. In this case, recurrence cases in the burning process are seen at a relatively low rate of 1-2%.
What are the Disadvantages of Warm Ablation Method?
Since warm ablation makes a more effective killing process in the target area, it can also cause some undesirable events. These are damages to the heart’s own electrical system. That is why the heart area where we will ablate is very important. If the heart point or region that we will destroy with ablation is a sensitive spot and the region where its own electrical system is concentrated, more care should be taken while performing the burning process. How to Perform Warm Ablation?
We can say that ablation is generally done with the same method as electrophysiological study (EPS). However, in ablation application, an additional catheter, “ablation catheter”, is used, unlike EPS. We can also say that ablation is the next stage of EPS. In most cases, the second step, ablation, is performed after the cause of the arrhythmia has been clarified with EPS.
Warm ablation application is the same as the general ablation application. Generally, veins in the inguinal region of the patient are used in ablation procedures. Depending on the type of rhythm disorder to be ablated, the patient is usually given a state of sleep, which we call sedation. Very rarely, general anesthesia can be needed if the patient is experiencing excessive tension from the procedure or if the procedure will take a long time. During the procedure, 1-2 mm thick cables called “catheters” are used to reach the heart. Stimulations are given to the heart through the catheters and thus the type of rhythm disorder present in the person is determined. In the meantime, the problem area is determined exactly, removed by applying warm application and the process is terminated. The duration of the warm ablation application can vary depending on the type of palpitation. However, it is a process that is generally completed within 1 hour. Most patients are discharged 24 hours after the procedure and patients can resume their daily lives.
